Who should sign an equipment handover form?

The person releasing the equipment and the person accepting it should both sign and date the form. The sender should be authorised to transfer the item, and the recipient should have had a reasonable opportunity to inspect it before accepting the recorded condition.

  • For employee equipment: the employee and an authorised manager, IT or asset custodian.
  • For rentals: the rental representative and the customer or authorised driver/operator.
  • For internal transfers: the current custodian and the new custodian, with approval if policy requires it.
  • For courier delivery: record the sender's dispatch evidence and the named recipient's acceptance.

When to add a witness

A witness is not necessary for every routine handover, but may be appropriate for high-value assets, disputed condition, handovers involving deposits, or where company policy or a contract requires one.
