What details must an equipment handover form include?

A complete form should identify both parties, the exact equipment, the date and place of transfer, its current condition, included accessories, the reason for the handover and both parties' acknowledgement. Use serial numbers or asset IDs so the record cannot be confused with a similar item.

  • Handover date, time and location.
  • Names, employee or company details and contact information for both parties.
  • Item description, make, model, serial number, asset ID or registration number.
  • Physical and functional condition, meter or odometer reading, and fuel or battery level where relevant.
  • Every accessory supplied, such as keys, chargers, cables, batteries, cases and manuals.
  • Purpose of the handover, expected return date and any restrictions on use.
  • Notes, photographs or inspection references for existing damage.
  • Dated signatures from the person handing over and the person receiving.

Use precise descriptions

Avoid descriptions such as “laptop in good condition” on their own. A stronger record names the item and states observable facts—for example, “Dell Latitude 5440, serial ABC123, powers on, screen intact, small scratch on rear-left corner, 65 W charger included.”
