What the form proves
The form creates a point-in-time record that the equipment was delivered, received and checked. It reduces disputes about whether an item was supplied, what condition it was in or which accessories accompanied it.
It is commonly used for employee onboarding and offboarding, internal transfers, contractor equipment, vehicle or tool allocation, rentals and returns.
What makes it useful
- The asset is identified by serial number, asset ID or registration number.
- The condition is agreed while both parties can inspect the item.
- Responsibility has a clear start date and, where relevant, a return date.
- Both parties keep the same written record of the exchange.
