Should PPE replacements be recorded?

Yes. A replacement record shows what was replaced, when and why while preserving the previous issue. Common reasons include normal wear, damage, contamination, loss, incorrect fit, expiry, failed inspection or a change in task or required protection.

Keep the previous issue in the history

Do not overwrite the original row so that it looks as though the employee always had the new PPE. Close or return the previous issue and create a new issue event for the replacement. This preserves dates, condition and reason for the change.

  • Damage, defect or failed inspection.
  • Contamination or exposure.
  • Lost or not returned.
  • Incorrect fit, changed task or changed specification.
  • Expiry or withdrawal from service.
